Schedule a medical consultation or follow-up :
Appointments can be booked at any time for patients treated at the Soultzmatt medical practice by calling the voice server at 03.89.47.67.94 - dial 1, or on this website after creating your account.
You can also make an appointment with the practice's secretariat * from 8.15am to 12.30pm, Monday to Friday, on 03.89.47.67.94.
Certain time slots are only available with the doctor's agreement (visits, long consultations, emergencies, etc.).
Please note: Patients who are not registered with the practice cannot make their own appointments. They should not create an account, but call the secretary's office.

For same-day emergency consultations, teleconsultations or messages, please contact the Doctor's Message Service ** during their working hours or the secretary * .

Notification policy
- After making an appointment (by the secretary or yourself), you will receive immediate confirmation, as soon as the reservation is made.
- Reminder the day before for all bookings made up to D-3
- A distant reminder (one only) allows you to reorganize:
- - if booking > 1 month: 1 week before
- if booking > 4 months: 2 weeks before
- if booking > 8 months: 3 weeks before
- Connection link 10min before teleconsultations
Please prepare for the appointment and bring:
- Your up-to-date administrative documents (ID, health insurance card, mutual insurance card, CMUc/AME/ACS certificate, long-term care protocol, work accident report, etc.)
- Your recent medical documents (letters, test results, prescriptions, questionnaires/statements requested by the doctor, etc.). You can submit these documents electronically within 7 days prior to the appointment by logging into your appointment booking account.
- Your child's health record (regardless of the reason for the appointment)
- The items required for the appointment: vaccine(s) + vaccination record, medication(s), etc.
- A means of payment for the appointment (cash, credit card, check)
* You can contact the secretariat during the opening hours of the secretariat from 8:15 a.m. to 12:30 p.m., telephone 03 89 47 67 94. You will be answered by a voice server who will direct you to the secretariat. -> Option 1 "make an appointment", if the secretariat is busy, you will be transferred to the voice server appointments (for patients followed in the office). -> Option 2 "Need to contact us", if the secretary is busy, you will be transferred to the doctors' messaging system (accessible during the doctors' working hours). ** You can leave a message to the doctors using the contact form (on the website - on each doctor's page) or using the voice server phone - option 2.
